Your order automatically heads to the top of our list. Shipping costs are calculated by weight and distance. (backorders are not included in these costs) Wholesale Account Requirements: Download Application (.pdf formated) A copy of your business license. A copy of your sales tax license. A copy of your business check. A copy of a business advertisement. (i.e. Stained glass yellow page listing, online yellow page listing, craft show vendor listing, class instructor listing, newspaper advertisement, sales flyer, or your website address) Each application will be individually reviewed. Please allow us at least 2 business days to verify your application and information. After your application has been approved, an initial purchase of $500 must be met.
